Patient Assessment

Individualized/personalized plan

LifeStream will develop a personalized plan of care tailored to the client's individualized needs. The personal plan will be based upon the initial assessment that focuses on the client's physical, emotional, mental & social needs. Short & long term goals along with strategies are defined and the entire plan is implemented.

Mental & emotional assessment

LifeStream will focus on the client's current ability to care for one's self. Among the factors considered will be the client's orientation to the surroundings and memory skills, along with the capability to carry out the activities of daily living.

Physical capability assessment

LifeStream will analyze the client's ability to function independently, in and outside of the home environment. A determination will be made as to whether any assistance is required whether it be ambulatory or transportation services. LifeStream will arrange all necessary transportation required for the client's needs. At all times, safety and comfort of the client are our utmost concern.

Physician consultation

LifeStream will accompany the client to their medical appointments. We are versed in gathering information on medical conditions and needs along with the medications prescribed. LifeStream will not only consult with the client's physicians but also explain to the client and their family all aspects of the medical treatment.

Financial assessment & recommendations

Often many of LifeStream's clients are unable to manage their own finances. LifeStream can assist in asset preservation and help the client manage their bills & expenses. LifeStream's unique affiliation with elder care attorneys also allows us to gather key and essential information on government entitlement programs such as Medicare; or assistance programs such as Medicaid and Social Security.

Hospital discharge service

When a client has been hospitalized, LifeStream can work, prior to the discharge, with the client, his family and health care team to ensure that all aspects of the hospital discharge run smoothly. LifeStream will gather all necessary information regarding the client's on going treatment, medications, correct dosages, future medical appointments, and arrange transportation back home from the hospital. Once settled back at home, LifeStream will set into action and implement all necessary steps to make sure that the client's medical discharge treatment plan set up by the client's physicians is continuously followed and adhered.

Nutritional assessment

LifeStream employs a full time nutritionist on our staff. Upon request, we will, in conjunction with the client's physician, analyze the client's diet. LifeStream's nutritionist will make recommendations to ensue that all the necessary vitamins and supplements are within the meal plan of the client.
