Care Manager Services
- care manager visits
Upon implementation of the individualized care plan, LifeStream will continue to provide support for the client through its on going services. The LifeStream care managers may schedule regular visits along with telephone calls to the client. In addition, the client's family can be kept informed with regular status reports.
- supervision & support of caregiver
LifeStream will supervise the activities of the daily care giver to ensure that the client is being given the best possible care and that all the client's needs under the individualized care plan are being satisfied. Often care givers are not aware of the changing needs of our client and we are there to inform them and see that the plan is carried out.
In addition, LifeStream can provide emotional support to the care giver that is overwhelmed and psychologically exhausted from the care giving process. LifeStream's counseling to the caregiver will not only help and support the care giver but will ultimately lead to better care of the client.
- home safety assessment
LifeStream's care managers will perform a complete inspection of the client's residence to ensure that it meets all safety requirements necessary for that individual in order for the client to be able to remain within the home setting. If there are any alterations that need to be made such as bathroom safety bars, LifeStream will immediately notify the client and arrange for necessary repair and installation.
- on-going monitoring
LifeStream will continue to provide support and see that the client is receiving the best care possible. An effective individualized care plan must be flexible and quite often must be amended with the changing needs and circumstances of the client. LifeStream will implement all the necessary changes and make sure that any transition takes place smoothly and effectively.
- access to neccessary services to stay at home
LifeStream will strive to enable a client to remain at home. We have experience and can recommend numerous services such as home care giving aids, companions and cleaning services to enable our client to remain within the comforts and familiarity of their own home.
- counseling
LifeStream's Robin Fried & Susan Elias are both New York State Licensed Certified Social Workers who have extensive experience in geriatric counseling. Their services are available to any client, caregiver or family member in need of counseling whether it be due to aging, transition or illness issues.
Ms. Fried & Ms. Elias have extensive experience in providing specialized counseling to the unique needs of older adults and their families. Their services are available to any client, family member or caregiver. Counseling often provides benefits during periods of crisis, transition or adaptation to physical limitations. Families often utilize counseling services to help discuss difficult issues regarding limiting an older person's autonomy such as giving up driving, cooking or accepting a home health aide or companion.
- referral to occupational and physical therapists and speech pathologists
Occupational and physical therapist along with speech pathologists can restore levels of functioning in elderly persons that will make it easier for the client to remain more independent and optimize mental and physical well-being. LifeStream has access to all the specialists in this area that would assist the client and improve the quality of life.
